How to Get Started
Most patients tell us our new patient process is unlike anything they have experienced in a healthcare setting. During the Case Review we dive into your whole story, what your birth was like, activities during childhood, past injuries, your career, diet, lifestyle. Dr. Mirandola wants to know and understand your whole story because this will be the “why” behind the examination results that are soon to follow.
During the Examination, 10-16 Structural, Neurological, and Physiological examinations create a baseline for the overall state of your neuro-structural health.
The Review of Results is where we weave all your test results together and create an easy to understand 360° picture of your current state of neuro-structural health which guides the care and adjustments we do.

What Happens
Your first interaction with The Foundation Chiropractic is a private conversation with Dr. Mirandola not a commitment.
During this consultation, Dr. Mirandola will:
Review your health history in detail — the challenges you are currently experiencing, how long they have been present, and what you have already tried. He will ask questions that most healthcare providers have never asked you before, because he is looking for the root of the issue rather than a description of its surface.
Answer your questions honestly — about our approach, the Gonstead Method, the examination process, the expected cost of care, and whether your specific situation is one he believes we can help with.
Determine whether we are the right fit — at the end of the case review, Dr. Mirandola will tell you directly whether he believes our approach can meaningfully address your situation. If he does not think we can help, he will tell you that clearly and may suggest where to look instead.
What to know
It is a genuine consultation — not a sales call. Its purpose is mutual: you evaluate whether The Foundation Chiropractic is the right fit for your needs, and Dr. Mirandola evaluates whether your situation is one he can genuinely help with.
If you decide after the case review that our approach is not what you are looking for, you are under no obligation to continue. If Dr. Mirandola determines that your situation is better served by a different provider or specialist, he will say so directly.
Most case reviews run 30–40 minutes. Some take longer if the history is complex. Come prepared with a clear description of what has brought you here.

What Happens
What to know
- Wear comfortable clothing that allows you to move freely. Patients are not required to change into a gown.
- Refrain from taking medications or caffeine prior to your examination.
- Refrain from exercising at least 3 hours prior to your appointment.
- Do NOT wear loose-fitting clothing Shorts or workout pants and t-shirt are ideal. Also, please bring socks, as we ask all members to kindly remove their shoes at our door
- Four ears are always better than two. Bring along anyone involved in your health decision making process.
- You will not receive an adjustment at this visit. The findings must be fully analyzed first. Dr. Mirandola will never adjust a patient he has not thoroughly examined or clearly identified as needing correction.

Review of Results & Your Care Plan

What Happens
After Dr. Mirandola has carefully analyzed your examination findings and X‑rays — typically within the week following your examination — you return for a full review of your results. At this appointment, Dr. Mirandola will:
Walk you through your X‑ray findings — using your actual imaging to show you what was identified, where in your spine the structural issues are present, and how they have contributed to the challenges you presented with.
Explain the clinical findings in plain language — not medical jargon designed to sound impressive, but honest, direct language designed to help you actually understand your own health.
Present your care plan in detail — the recommended schedule, the expected timeline for improvement, the milestones used to assess progress, and the total investment required.
Begin care at this visit — if you choose to proceed. If you decide to move forward, your first adjustment takes place at this appointment.
What to know
You will never be pressured to make a decision at this visit. The Review of Results is your information session. You may need time to consider the findings, discuss with a partner, or review the financial investment. That is completely understood and respected.
The care plan is yours — not ours. Dr. Mirandola will present a recommendation based on your examination findings and health goals. You decide whether and when to begin. Patients who begin care because they understand and believe in the plan get better results than those who feel pressured into it.
Most patients choose to begin care at this visit because by the time they reach the Review of Results, they have a thorough understanding of what was found and a clear picture of what addressing it will require. The decision feels informed rather than impulsive.

We are out‑of‑network with all carriers. Here is exactly why, and what that means for you.
You invest a great deal in your health insurance each year, so it’s completely understandable to want to use those benefits whenever possible. We truly respect that.
When we first opened in 2010, we did accept insurance. Over time, however, we began to see that insurance guidelines often limited the type of care we could provide and the results our patients were capable of achieving. Decisions about frequency of visits or length of care were sometimes dictated by coverage rules rather than by what a person genuinely needed.
We also found that many patients were still facing high co-pays or large deductibles, sometimes paying nearly the full cost of care out of pocket before insurance contributed at all.
After careful consideration, we made the decision to move out-of-network with all insurance carriers. This was not a business decision — it was a clinical one. We wanted to protect the integrity of our examination process and ensure that every care plan we recommend is based solely on your individual health, not on insurance limitations.
HSA & FSA accepted
We accept Health Savings Account (HSA) and Flexible Spending Account (FSA) payments for all services. Chiropractic care is an IRS‑qualified medical expense — which means you can use pre‑tax dollars to cover the full cost of your New Patient Experience and ongoing care. Many patients find that HSA/FSA coverage makes our care more accessible than they expected.
Out‑of‑network reimbursement
If your insurance plan includes out‑of‑network chiropractic benefits, we provide an itemized superbill that you can submit directly to your carrier for potential reimbursement. Many patients recover a meaningful portion of their care cost this way. We encourage you to call your insurance provider before your first visit to ask about your out‑of‑network benefits.

Here are the four things most new patients want to know.
How do I schedule my first appointment?
The first step is scheduling an online Case Review with Dr. Mirandola — a 30–40 minute video or phone consultation at no charge. You can schedule this by calling either of our locations directly. Once the Case Review is complete and Dr. Mirandola confirms that our approach is a good fit, you will schedule your in‑office Neuro‑Structural Examination at your preferred location — Wellesley or Worcester.
In practice, the majority of adults who present for examination have some structural finding worth addressing. But “worth addressing” exists on a spectrum — from minor functional improvements to significant corrections of longstanding dysfunction. Whatever the findings, they will be presented to you accurately and honestly, and the recommended course of action will reflect what the examination actually shows.
I've seen a chiropractor before. Do I have to go through the whole examination process again?
Yes — and for a specific reason. Our examination is not a standard intake process. It is a 10–16 point Neuro‑Structural Examination that includes Gonstead‑specific instrumentation, full‑spine X‑rays taken in the standing weight‑bearing position, and a level of analysis that most previous chiropractic assessments have not performed.
Previous X‑rays, if you have them, are welcome to bring and may provide useful context — but they are typically not taken in the same position or with the same structural focus our analysis requires. The examination Dr. Mirandola performs is the foundation of the care he recommends. It cannot be substituted by what a different provider found using a different methodology.
What if the examination doesn't find anything significant?
That is a completely valid outcome — and one we welcome. If the Gonstead examination does not reveal structural interference significant enough to warrant a corrective care plan, Dr. Mirandola will tell you that directly. You will not be sold a care plan you do not need.

How long will I actually need to come in? I don't want to be committed to something open‑ended.
Your care plan has a defined structure — not an open‑ended commitment. At the Review of Results, Dr. Mirandola presents a specific recommended schedule with a specific timeline and a specific set of milestones he will use to assess your progress. You always know where you are in the plan and what the next checkpoint is.
The length of care depends on the severity and duration of the structural issue, your overall health, and how consistently you attend care. What we will never do is recommend indefinite care without a clear structural rationale. If your structural findings warrant it, we will tell you why. If they do not, we will tell you that too.
I'm nervous about chiropractic adjustments. What if I don't respond well?
The nervousness is understandable — and it is one of the reasons we invest so much in the examination and Review of Results before any adjustment is made. Dr. Mirandola will not adjust a segment he has not clearly identified as needing correction, in a direction he has not confirmed through five‑criteria analysis. The Gonstead Method’s precision significantly reduces the discomfort and uncertainty associated with more general chiropractic techniques.
Before your first adjustment, Dr. Mirandola will explain exactly what he plans to do, what you will feel, and what to expect afterward. If at any point you have concerns, communicate them — the approach can always be modified to your comfort level. We have never had a patient who could not be cared for within a comfortable range of pressure and technique.
Can I bring someone to my appointments?
Yes — and we encourage it, particularly for the Review of Results appointment. Having a spouse, partner, or trusted family member present when Dr. Mirandola presents your findings and care plan recommendations means a second set of ears, someone to ask questions on your behalf, and a shared understanding of what the care involves. For pediatric patients, parents are always present and are actively included in the examination and review process.
Will Dr. Mirandola be the only chiropractor I see?
Yes. The Foundation Chiropractic is a focused, single‑practitioner practice. Dr. Mirandola performs every examination, every Review of Results, and every adjustment personally. There are no associates, no handoffs, and no adjustments delivered by someone who did not perform your examination. This is a deliberate choice — the Gonstead Method’s precision is built on the assumption that the same practitioner who performed the analysis delivers the correction. Continuity of care is not a feature here; it is a structural requirement of the method itself.
